Welcome to the 2019 Benefits Program!
Hays Companies offers a full suite of benefits including medical, dental, vision, disability, life and AD&D, wellness programs, and value added services. This benefits summary provides highlights of these plans and programs. Please read the summary plan descriptions and additional documents for full plan information.
Who is Eligible?
All full-time and part-time employees are eligible to participate in medical, dental, vision, disability insurance, and dependent care FSA on their date of hire. You may also cover your eligible dependents. Life insurance and AD&D eligibility begins on the first of the month following 30 days of employment.
Any elections made during the Initial Enrollment are in effect for the plan year and cannot be changed without experiencing a “qualified life event” such as:
- Marriage, Divorce, Legal Separation
- Birth or Adoption of a child
- Change in child’s dependent status
- Death of spouse, child, or other qualified dependent
- Change in spouse’s employment status
- Involuntary loss of other coverage
XYZ Companies Benefits Summary
XYZ Companies, Inc. recognizes the importance of offering a comprehensive benefits program to all eligible employees working at least 32 hours per week. You are eligible for benefits on the first of the month following 30 days of continuous employment. These benefits help provide employees and their family members opportunities to maintain their health and welfare. In this “Benefits Summary,” employees will be able to review:

- An outline of Medical, Dental, Life, AD&D, Voluntary STD, LTD plans, and much more
- Important contact information and websites to help employees manage their benefits
The purpose of the XYZ Companies Benefits Summary is to highlight the major aspects of our benefits so that it may be a quick reference tool for you. A full description of each benefit plan can be found in the summary plan descriptions (SPD), which takes precedence over the XYZ Companies Benefits Summary. You will receive your SPDs after electing your benefit choices, and they will be posted on the XYZ Companies intranet as well. Please review this to understand your benefit enrollment options, and please retain this XYZ Companies Benefits Summary for reference throughout the year.
