Overview
XYZ Companies provides a Basic Life Insurance policy through Mutual of Omaha of 2x times your annual salary (up to a maximum of $200,000) at no cost to you.
Accidental Death and Dismemberment (AD&D) coverage is also provided up to the amount of your life benefit.
Important Note
Your Life and AD&D benefits are paid to the individual noted on your Beneficiary Form.
If you need to update your beneficiary information, you will need to submit a new form to XYZ Companies Resources.
